Pregnancy Modification Request Form
Title IX prohibits discrimination based on sex in education programs or activities, which includes prohibiting discrimination on the basis of sex—including pregnancy and pregnancy related conditions—in educational programs and activities. Pregnant students or employees may ask for pregnancy modifications from the Civil Rights and Equity Office due to pregnancy, childbirth, and related conditions. This covers instances of a pregnancy, childbirth, false pregnancy, termination of pregnancy, or recovery therefrom. More information can be found on the U.S. Department of Education website.
