Incident Reporting

All workplace incidents, whether they result in an injury or illness or not, regardless of severity, are required to be reported within 24 hours.

For more information on reporting workplace incidents, or questions regarding Workers' Compensation please visit the Human Resources website.

Incidents may be reported online through the UNH Incident Report form.

New Hampshire Department of Labor Notification

The State of New Hampshire Department of Labor, under the Lab rule chapter 1403.04, "Accident Reporting Requirements," requires within eight hours after its occurrence, employers report an employee accident which is fatal to one or more employees or which results in the hospitalization of three or more employees, to be reported to the Commissioner of Labor.  Contact information for reporting these incidents is included in the chapter.

The Office of Environmental Health and Safety (OEHS) and the UNH Office of Emergency Management have established procedures to ensure the proper notification of all workplace accidents as described above.  All questions should be directed to OEHS.
