Use the Safe Electronic Equipment Disposal Form to enter disposal requests. You will receive an email from the SEED program manager when your request is approved NOTE: Your request must be entered no later than two weeks prior to the scheduled collection date to insure capacity for your equipment.
Start by entering your Contact Information. IT will use this information to contact you to resolve any questions on your request and to notify you as to the collection date and times for your approved request.
Enter the Equipment Information for the electronic item(s) to be dispositioned. You can enter items singly or use the template provided to enter multiple pieces of equipment at one time.
For single entries, click on that button on the form:
Select the type of Equipment from the drop-down box and enter the required information into the displayed fields. All single entries include a comment field which is optional.
For Monitors, please enter the Type (CRT or flat) and the Size (15 inch, 19 inch, etc.) into the Description
For Peripherals and Other, please enter the Quantity into the Description, if more than one (for example, 5 keyboards, 6 cables, etc.)
For Other, if entering a device which has data storage (such as a hard drive), please include the serial number in the Description.
When a single entry is complete, click on the ‘Add to List’ button. The form will display all single equipment entries you have made to this point. If you have not entered all required fields, the page will display an error message.
If you need to change any entry, click on the ‘Edit’ button beside that entry.
When you have completed the edit, click again on the ‘Add to List’ button.
When you have added all your equipment to the list, and you have reviewed the entries for accuracy, click on the ‘Submit’ button at the bottom of the page to submit the request.
To enter information using the template (a csv spreadsheet), click on that button:
Follow the instructions on the form to download and save the template as a csv file type.
Open the downloaded template and add the equipment information. The matrix below shows the required information for each Item in the template.
Review the information for accuracy, save the file as a csv file type and follow the instructions on the web form to upload the template.
When the template has been uploaded, click on the ‘Submit’ button at the bottom of the page to submit your request.
To clear all information fields on the page at any time before the request is submitted, click on the ‘Reset’ button at the bottom of the page. You will have to re-enter all fields, or a template, to submit a request.
