Send copy of resignation letter to BSC. BSC will enter termination date in Banner.

|Pcard returned to IT BSC, IT BSC will cancel and destroy.|
|Spare Laptop (including peripherals) returned.|
Time & Attendance sheet signed and submitted. Send an original signed timesheet to Human Resources with a copy to BSC for vacation time payout when the employee terminates.
|UNH ID Card returned.|
Remove employee from IT Staff Distribution Lists, supervisor to contact AVP
Administrative Assistant to remove the employee from appropriate IT - wide distribution lists.
|Parking Notified (862-1010) of Termination.|
