Change in Personal Information Request

Address Change

  1. Come to the Registrar's Office in Stoke Hall and fill out a Change of Address Form;
  2. Or, download and fill out the Change of Address Form or send a written request containing:  name, ID number, old and new addresses, and signature. Mail it to the UNH Registrar's Office, 11 Garrison Avenue, Durham, NH  03824-3511 or you may e-mail it from your UNH e-mail account to registrars.office@unh.edu.

Name Change

  1. Come to the Registrar's Office in Stoke Hall and fill out a Change of Name Form;
  2. Or, download and fill out a Change of Name Form. If you are a current student you must attach a copy of your Social Security Card with your new name in order for the change to be made.
  3. Bring, fax or mail the form to the UNH Registrar's Office, Stoke Hall, 11 Garrison Avenue, Durham, NH  03824-3511. Fax to (603) 862-1817. Do not e-mail documents containing your Social Security number.
