How Do I Change My Name?

    There are two ways to change your name:

1)  Come to the Registration Office in Stoke Hall and fill out a Change of Name Form.

2)  Either fill out a Change of Name Form or submit a written request containing:  Old and new name, ID number, signature and mail it to the UNH Registrar's Office, 11 Garrison Avenue, Durham, NH  03824-3511.
