INDEPENDENT CONTRACTOR AGREEMENT

The Independent Contractor Agreement contains instructions for hiring an independent contractor and detailed instructions on entering a requisition. To determine whether an individual should be considered an employee or independent contractor, refer to the Employee vs. Independent Contractor Determination Checklist included with the agreement.
