PIs/PDs are responsible for initiating action, submitting an application or registration, or otherwise following up for all items checked “Yes” in Section 14 of the Yellow Sheet.

For example, if the project would involve shipments out of the U.S., the PI/PD must consult UNH’s Export Controls Management Plan. If the project would involve human subjects, the PI/PD must prepare an application for submission to the UNH Institutional Review Board for the Protection of Human Subjects in Research. If the PI/PD proposes to include project personnel outside the PI’s/PD’s organizational unit, the PI/PD is responsible for ensuring that such personnel complete the "Additional UNH Faculty/Staff” form with the Yellow Sheet.
