Instructions for Standard Information Sheet

All steps on this sheet are data enterable.

[1] Enter Your Name Enter your first and last name.
[2] Date of Hire Enter the date of your original date of hire.
[3] Department Enter your department.
[4] Your Phone number Enter your campus phone number or a number that you can be reached at during business hours.
[5] End date of the first Payperiod this year Enter the Payperiod ending date of the first Entry Period. Refer to the Normal Work Hours sheet for a listing of the Payperiods for the current fiscal year and the ending dates of those Payperiods. For example, if you are using the form for the first time on June 30, 2008, then the Payperiod end date of your first Entry Period would be July 11, 2008.
[6] Timesheet Org. This is the 6-digit number that reflects your department name. It's a Banner attribute assigned on the NBAJOBS record that associates an employee with a department or organization for the purposes of facilitating payroll processes. (If you don't know your Timesheet Org., you can get it from your BSC Contact). NOTE: Completion of this field will not be mandatory until recording of time and leave in Banner begins.
[7] Position - Suffix A two character code that provides uniqueness of a specific appointment for the individual and position number. Example: the first occurrence will set the suffix to 00, the second to 01, etc. (If you don't know your Position - Suffix, you can get it from your BSC Contact). NOTE: Completion of this field will not be mandatory until recording of time and leave in Banner begins.
[8] Comp Time Enter the Comp Time accured prior to the beginning date of the first Entry period.
[9] Earned Time/Vacation Enter the Earned Time/Vacation accured prior to the beginning date of the first Entry period. If this is the beginning of the fiscal year, then enter your Earned Time from the end of the prior fiscal year.
[10] Sick Pool/Sick Leave Enter the Sick Pool/Sick Leave accured prior to the beginning date of the first Entry period. If this is the beginning of the fiscal year, then enter your Sick Pool from the end of the prior fiscal year.
[11] Normal Works Hours Enter a Y in the field next to the schedule that you primarily work. If you work a schedule other than one that is listed, then refer to Step 10.
[12] Other work schedule If you work a schedule other than one listed above, then enter Y here.
[13] Other work schedule If you entered Y in Step 10, then enter your work schedule here. For example, Monday - Saturday 5:00 AM to 2:30 PM
[14] Lunch Period Enter your normal lunch period here. For example, 12:00 PM to 1:00 PM.
