Instructions to Request Background Review
Step 1: Download the Information Disclosure and Consent Form.
Step 2: The hiring department completes the top section of the Information Disclosure and Consent Form and selects appropriate background screening packet, contact name and number.
Step 3: Have applicant complete the bottom section of the Information Disclosure and Consent Form and give applicant a copy of Summary of Rights according to FCRA. (Applicant to keep for their records, do not submit to HR).
Step 4: The hiring department sends the completed Information Disclosure and Consent Form (and attached copy of candidate’s CV for faculty appointments) to HR for process by one of following options: Send via email to: Mariah.email@example.com or fax to: (603) 862-1222.
Motor Vehicle Records
Candidate’s that require a NH Motor Vehicle search in addition to the appropriate consent form must also complete a NH Release of Motor Vehicle Record Form. Candidates are required to complete section V & VI of the form and have it signed in the presence of duly authorized Notary Public. The ORIGINAL 2-part form is sent to Mariah Bellington at the Office of Human Resources, 2 Leavitt Lane, Durham, NH 03824.
