Eye Care

Employees who select UNH's Harvard Pilgrim plan as your healthcare provider are now entitled to an eye exam on an annual basis with the optometrist of your choice. (effective January, 2004).

For those employees whose healthcare coverage provides for bi-annual eye exams only, a complete eye exam can be obtained for the alternating year through a seacoast optometrist for a fee of $7.00. The participating provider is:

1. Applications are available from UNH Human Resources, 2 Leavitt Lane, between 8:30 a.m. and 4:00 p.m.
   
   
2. After paying $7.00 (check payable to UNH), employee schedules an appointment with Dr. Menard.



3. Eligible employees may take advantage of this arrangement once every two years. The exams are available year-round on a first-come-first serve basis.

Dr. Norman Menard's office also provides certain eye care services and products to UNH employees, retirees, spouses and dependent children at discounted rates. To take advantage of this discount, the employee must show a valid University ID card at the time of the appointment AND pay for the services and products at the time they are received.

Also:

LaFreniere Eyecare, located in downtown Durham, offers USNH employees, their dependents, and retirees preferred provider rates for exams and services. They offer a comprehensive eye exam, which includes a PRIO computer vision exam. Your cost is the insurance co-pay or $15.00 if there is no insurance to bill. Eyeglasses are discounted 5%, contact lenses 5%, and other non-RX items at 5%.

Pearle Vision Center, located in Dover, NH offers UNH employee, family (spouse and children), retiree and student discount on exams, glasses, contact lenses and accessories (non-Rx items). Discounts are as follows: Eye exams: $10 Discount; Eyeglasses: 35% Discount; Contacts: 10% Discount; Non-Rx Items: 20% Discount.
