Forms to Complete

  • Senior Checklist -- This form should be completed by you in either the second semester of your junior year or the first semester of your senior year. It can be filled in online, downloaded or printed from the link provided or you can pick up a copy in the UHP office. Completed forms need to be returned to the UHP office.


  • Certification of Completion of Honors Program Requirements -- This form is to be completed by you and signed by your honors departmental liaison at the beginning of spring semester and is proof that you are in good standing for graduating with an honors designation. Print the form from the link provided or pick up a copy in the UHP office. Completed forms need to be returned to the UHP office. Please be advised that if we do not receive this form from you prior to our deadline then you will not receive an honors designation upon graduation.
