Leave of Absence

Note: The following information is for undergraduate students who are seeking a leave of absence for health (medical or psychological) reasons. Graduate students should call or email Cari Moorhead, Ph.D. Associate Dean, UNH Graduate School.

Process and Policy

During the fall and spring semesters, when classes are in session, requests for a leave of absence from the University for health (medical or psychological) reasons are initiated by contacting the Assistant Vice President for Student Life/Executive Director of Health & Wellness, Dr. Kevin Charles. Contact him by phone at 603-862-1098 or contact him by email.

After classes end, you must contact your college’s Associate Dean. If you receive any financial aid, the effective date of your leave of absence may affect your present and/or future financial aid.  Please check with Business Services by calling 603-862-2230.

Leave of Absence Letter

As soon as possible after communication with Dr. Charles at Health & Wellness, you will need to request a letter from your health care provider (physician, therapist, etc.) to support your leave of absence request. This confidential information will remain at Health & Wellness. View a sample leave of absence letter

This letter must:

  • be typed on office letterhead stationery
  • indicate the diagnosis and/or specific nature of your health condition
  • verify and support that as the reason for your need to take a leave of absence from UNH

Have your health care provider mail or fax the letter to:

Housing & Residential Life

If you live on campus, check out of your residence hall/apartment, leave your key with the Hall Director or Assistant Hall Director, and sign off on your room inventory form. This process will determine your official check-out date from your residence. Any possible housing refund is calculated according to that date.

If you presently live on campus and will require on-campus housing upon your intended return to UNH, contact Housing & Residential Life. If your condition will require any physical modifications to your room/bathroom, you must notify Housing & Residential Life as well as Student Accessibility Services at least 12 weeks prior to your intended arrival.

You will also need to print and complete the Request Form, and mail, scan, or fax it back to Dr. Charles. Please note that UNH policy states that students are prohibited from returning until one semester has elapsed following a leave of absence for health reasons.

Dining

If you have a Dining (meals) plan, contact Dining regarding any potential refund.

Insurance & Student Health Benefits Plan

You are responsible for determining how your leave of absence may affect your health insurance coverage. If you have the UNH Student Health Benefits Plan (SHBP), please contact Cindy McGahey, at 603-862-2853, with any questions.

Returning to UNH

Every student who plans to return to the University must notify the Registrar's Office and must be cleared by the Executive Director of Health & Wellness (or designee) before returning from a leave of absence for health reasons.

Intent to Return Letter

You must obtain a letter from your health care provider who has treated you since your departure. The health care provider's information will be confidentially reviewed by the Executive Director of Health & Wellness. View a sample return to UNH letter.

This letter must indicate:

  • the extent of your treatment and the provider's assessment of your readiness to resume studies
  • a description of the current status of your condition
  • any special needs you may require upon your arrival on campus
