Required Health Forms and Information for Incoming Students

Attention GRADUATE STUDENTS: A Physical Assessment is not required for graduate students unless you are planning to participate in athletics or club sports, but the Immunizations and Health History Form are mandatory.
Section 1
Please print out the Physical Assessment and Immunization Record and have your health care provider complete and sign both forms. These forms must be mailed to Health Services, Attention Medical Records, 12 Ballard Street, Durham, NH 03824.
- Physical Assessment (Required) - Click here to print. First year students are required to have a physical exam upon entrance into the University. We understand that some students may have had a physical exam within the year and insurance companies will not pay for a second exam. If this is the case, please have your health care provider complete the Physical Assessment form with the date and information from your recent exam. Students intending to play a sport should contact their coach for specific physical date requirements.

Section 2
HEALTH HISTORY FORM - This form will not be available to new students until June 15, 2008. Please check back.
- Health History Form
- Students are required to complete an online Health History Form.
- Students will need both the completed Physical Assessment and Immunization Record to assist them with completion of the online Health History Form.
- To access this online form you need to have your UNH username (also known as IT ID or Blackboard ID) and password. If you experience problems logging in or don't know your UNH username, please contact the CIS Helpdesk at (603) 862-4242.

A medical record is created when Health Services receives a student's Physical Assessment, Immunization Record and Health History Form, or upon his/her first visit to Health Services. For students who live with chronic illnesses, it may be helpful to submit additional health information to be made part of their medical record to assist with continuity of care.
Student Health Insurance
Effective for the 2007-08 academic year, the University of New Hampshire will be joining a growing number of the nation's leading land grant universities in requiring health insurance as a condition of enrollment for all full-time students. The implementation of the requirement follows a year-long process of surveying students' health care needs, considering program objectives in consultation with students, parents, faculty, and professional staff, and conducting a comprehensive request-for-proposals process.
The University will be sponsoring a new UNH Student Health Benefits Plan administered by Health Plans, Inc., a subsidiary of Harvard-Pilgrim Health Care.
