How do I Qualify for Services?

1. To qualify for services, you must provide documentation of your disability as determined by a licensed physician and/or certified psychologist who is skilled in the diagnosis of such a disability.
2. Documentation must be current (generally within 3 years). Detailed Guidelines are available at Disability Services for Students for LD, ADD/HD, and Medical Disabilities. The Guidelines will enable you to work with Disability Services for Students and your physician or psychologist to provide appropriate documentation.
3. Once you have provided documentation, make an appointment with the staff to review your documentation, determine your eligibility, and discuss the accommodations, services, and referrals you need. You may meet in person, or on the phone if that is more convenient.
