Application Instructions
The COLA Study Abroad Application form is the standard application for all UNH COLA-managed study abroad programs (view list here). In addition to this standard form, each program requires its own supplement form, which is available on the specific program's website. All application materials should be submitted to the COLA Center for Study Abroad:

Study Away Student Eligibility form. This form is for UNH students only. If you are a non-UNH student, you should use the Non-UNH Study Away Student Eligibility Form.
COLA Study Abroad Application form. This is the general application form for all UNH-managed study abroad program administered through the COLA Center for Study Abroad. Please complete the form online.
Program Supplement form. Be sure to complete this program-specific form, which includes additional questions particular to your intended program.
Two passport-sized photos. A passport-sized ID photos is a 2" by 2" headshot. These photos can be taken in most pharmacies (Rite Aid, CVS, Walgreens, Wal-Mart, etc.), US Post Offices, or at UNH Photographic Services in Dimond Library Level G. Be sure to write your full name on the back of each photo.
Scan of passport bio page. Unlike the "passport-sized photo," this is a scan or photocopy of the bio/data page of your passport. If you do not have a passport, mark "pending" in the Passport Information section of the COLA Study Abroad Application form. You are strongly encouraged to apply for one immediately. Visit http://travel.state.gov/passport/ or a US Post Office to being the process.
Unofficial transcript. Submit your unofficial or official transcript. Students who have taken college-level courses at any institution other than UNH must submit unofficial or official transcripts of all such courses.
Reference form(s). Reference forms may not be required by all programs. Check the program's website to find out if there are any requirements in addition to this list.
Application fee. Check the program's website to find out if it requires an application fee. Application fees should be submitted by cash (exact change only) or check (made out to University of New Hampshire).
