Obtaining Copies of the JVQ

There are two versions of the JVQ (i.e., Caregiver and Child Self- report) and they both contain follow-up items that collect incident details such as perpetrator identity, occurrence of injury, and other information. If you would like to receive a copy of one, or both of the questionnaires, please fill out a permission form (indicate which version you would like) and return it to:
