ELDER AND FAMILY SUPPORT SERVICES FOR ALZHEIMER'S DISEASE AND OTHER MEMORY DISORDERS

EFSS is a program being conducted by the University of New Hampshire Department of Nursing. Its purpose is to assist elders and their families in anticipating, planning, and adjusting to the many issues associated with memory changes and cognitive impairment due to Alzheimer's Disease or other illness.

A nurse practitioner and a social worker will meet with you in your home or their office and provide you with information about the disease and answer your questions. They will provide you with specific resources in your area and assist you in creating a plan of care.

This program is available to residents of Rockingham and Strafford Counties.
